Sophia Romano is a top college student until a truck hits her. She wakes in a mafia world as a version of Sophia who borrowed from loan sharks for her mother's treatment and cannot repay. The debt is due, and Victor Falcone, the Falcone family Godfather, orders her taken away and dealt with by their rules.
Then she hears baby Luca Falcone's thoughts. Luca is crying because a wire inside his sock is digging into his skin. Sophia says she can stop him. Victor gives her one chance. She removes the wire. Luca smiles. Victor makes Sophia Luca's personal nanny at twenty thousand dollars a week.
Martha Colombo, the former childcare supervisor, is jealous of Sophia. She tests Sophia on exact milk temperature and bottle angle, threatening her finger if Luca cries. When Luca's tie chokes him, Sophia digs through his closet for something else to put on him. Martha has her held down and dismisses Luca's distress as nonsense. Victor says he saw everything, has Martha punished, and warns her. He then promotes Sophia to head nanny and doubles her pay.
Isabella hosts a parenting tea party for other crime families. Sophia takes Luca to the bulletproof play area. Martha sends the guards and maids away, then locks the play area with the children inside. The children are trapped without ventilation. Martha frames Sophia, claiming Sophia drove everyone off and locked the door to take credit.
The mothers panic and threaten war if their children are harmed. Sophia stays with the children. She ignores mob pride and family rules, calming them with gentle care. The children gather around Sophia and listen quietly as she tells them a story. Victor orders Martha to open the play area with the backup card. The children are safe. The other families offer Sophia money, penthouses, and a gunship to leave with them.
Victor refuses. Sophia belongs to the Falcone estate. He confronts Martha, who did all this, and has her punished by family law. Luca clings to Sophia. Victor declares Sophia the Falcone family's only head nanny, and anyone who tries to take her answers to his gun. Later, Victor breaks his rules for Sophia, and Arbiter Alexander grows obsessed. The choice between them is still open.
