Martina Hayes completes her final mission and retires. Her daughter Claire and son-in-law Cain, both FBI agents, are killed in the line of duty. Daisy, their daughter, is left in Martina's care. Martina buries her past and becomes a grandmother first.
Three years later, the agency asks her back. The Silver Gun Syndicate is regrouping, and Sierra Carter, the agency director and Martina's protégé, wants her to return. Martina refuses. She only wants to live quietly with Daisy. Ethan Miller, Sierra's deputy and senior field agent, is assigned to guard the family.
At school, Jax Harrington, the mayor's son, and his crew target Daisy. The bullying escalates to a locker-room attack. Jax threatens Martina and raises a baseball bat. Before the bat lands, Ethan Miller appears as Officer Miller, slaps Jax, and kneels to ask Martina's forgiveness. He was already assigned to protect the family. Jax and his crew are forced to apologize. They leave humiliated, not reformed.
Jax retaliates with his crew. They kidnap Daisy, assault her, and live-stream the horror to Martina. The agency locates Daisy. Martina refuses backup and takes the agency's top-tier gear. She reaches Daisy, but Daisy has multiple fractures and internal bleeding and is rushed into surgery.
Martina learns Jax is at his father Vaughn Harrington's mayoral victory party on a private island. She goes alone. A transport plane carries a tank; the tank drops from the sky, smashes through the banquet hall's glass roof, and crashes into the hall. Martina opens the hatch and calls out: "Jax Harrington, come out and face your death."
