Stephanie Sims has spent sixteen years carrying guilt for her mother's death. Her father Morris never looked at her, and her brother Alexander grew to despise her. The only warmth she remembers is Alexander giving her candy and telling her not to cry. Five years ago, their cousin Abigail Dawn moved in after losing her parents.

Abigail broke their mother's necklace and blamed Stephanie. Alexander believed her and called Stephanie a freak. Stephanie became the thief and the bully the family talked about.

Now Stephanie is diagnosed with stage four cancer and given three months without aggressive treatment. She calls Alexander to tell him. He orders her home for their father's birthday and hangs up. She looks at his photo on her phone and speaks to it: she is dying, and it will probably make his day.

At the party, she brings a cake. Alexander accuses her of copying Abigail and smashes the cake in her face.

Stephanie tries to buy a burial plot. Morris refuses to sign the paperwork and sends her to Alexander. At school, Alexander gives Stephanie's Ivy League summer program spot to Abigail. Stephanie drops out. Abigail provokes her, then frames her for choking Abigail. Alexander believes Abigail and demands an apology.

Stephanie is hospitalized for self-harm, and her cancer worsens. Dr. Smith calls Morris, who refuses to visit and would rather tend a dead body. Stephanie gets a tattoo to cover her scars. Alexander discovers it, forces her arm to see, and she hits him.

At prom, Abigail sends Jimmy to harass Stephanie. Stephanie pulls out a knife to keep him away. Abigail appears, cuts her own hand with the knife, and frames Stephanie. Alexander pushes Stephanie down and demands an apology. She refuses. He pours wine on her head. "Your little sister's dying, Alex. Are you happy now?" He orders her to shut up and blocks her exit. He grabs her hair and pulls it off, revealing her bald head.

After Stephanie dies, Alexander's world begins to fall apart.
