Winnie works at a hotel front desk and raises her son Cass alone. Three years after her breakup with Enzo, he walks into the hotel with another woman, Vivian. He orders Winnie to change his sheets, though she works the front desk. When she does, he confronts her about betraying him.

Five years earlier, Enzo—Don Moretti, head of New York's biggest mafia family—was attacked during a family sit-down. A bullet destroyed his kidney. He needed a transplant. Winnie offered to be tested. Enzo told her not to do it. She promised to do whatever it took to keep him alive. His father's bastards were making moves for control of the Moretti family, and Enzo told her to leave New York for safety. He wrote his will, leaving everything to her.

Now, Vivian accuses Winnie of stealing her diamond earrings and demands to search her. Winnie refuses. Enzo sends Vivian away and drives Winnie home. When they arrive, Enzo sees Marco, a man cast out of the Moretti family, waiting for her. Enzo mocks Marco for never buying Winnie anything decent, for taking a slice of the inheritance, for not even leaving her a parting gift. Then Enzo kisses Winnie in the car and tells her to get out.

Marco offers to explain things to Enzo. Winnie says she does not care if Enzo comes after her for it, and that she and Enzo ended three years ago. Later, Cass sees Enzo smoking on the street, runs over, and hugs his leg, calling him Daddy. Enzo does not know the child is his.
